A new Integrated Community Pulmonary Rehabilitation and Home Oxygen Assessment Service in Hounslow has created an exciting opportunity for a Service Lead (NHS Band 7 equivalent). Working for an international provider of Respiratory Healthcare Services, you will immediately become a member of our successful, growing Homecare team and part of a network of clinical specialists delivering our services across the UK.
Reporting to the Business Manager, COPD Services and working from a variety of community locations in Hounslow, your key responsibilities include:
To be the Service Lead:
• Lead development of the service
• Communicate and promote the service locally
• Ensure the service is highly integrated within the care pathway
• Together with the Business Manager, attend quarterly service review meetings with the Commissioners, ensuring that actions and activities required by NHS Hounslow are completed in a timely manner
• Propose and implement continuous improvements to the service
• Provide leadership for the team and clinical training and support
Provide evidence based Pulmonary Rehabilitation to patients by:
• Identification and recruitment of suitable patients to the programme
• Conducting initial and final discharge assessments
• Leading Pulmonary Rehabilitation supervised exercise sessions
• Presenting patient education sessions on a range of topics
• Providing discharge reports to the patient and the patients GP
Provide evidence based Home Oxygen Assessment and Review by:
• Assessing existing home oxygen patients and all new patients referred
• Ensure systems are in place to effectively pre-screen patients
• Undertake capillary blood gas sampling and accurately interpret the results
• Conduct ambulatory oxygen assessment
• Ensure cost effective appropriate prescribing
• Conduct follow up appointments
• Educate patients and their carers on the safe and effective use of home oxygen therapy
Provide a community-based respiratory care service by:
• Supporting the Community Matron Team with specialist care to COPD patients
• Liaison with acute providers to identify COPD patients who are being discharged and ensure all these patients are contacted by the service
• Reduce incidence of hospital admission due to exacerbation by leading and providing support to patients who are deteriorating by visit and by telephone
• Provide self management support and education to patients
Ensure integration with other local Health Care teams and professionals
Develop the service to be recognised as a source of Respiratory Expertise to all clinicians within NHS Hounslow
Support and adhere to our Clinical and Information Governance programmes
REQUIRED KEY SKILLS
• Arterial/capillary blood gas sampling, measurement and interpretation
• Conducting walk tests within respiratory care
• Spirometry and interpretation
• Computer and electronic patient management software
• Knowledge and understanding of relevant national and international guidelines
• Excellent communication and interpersonal skills
• High patient orientation
• Ability to act on own initiative and take responsibility of own caseload
• To be highly motivated and highly motivating
• Desire to constantly improve self and the service
QUALIFICATIONS:
• National qualification (Registered Nurse on part 1 or 2 of the Nursing and midwife Council register) with current NMC registration or Registered Physiotherapist with HCP registration
• ENB 998 or equivalent (teaching and assessing in Clinical Practice post graduate module)
• COPD respiratory diploma or other recognised respiratory course
• Evidence of continued professional development
EXPERIENCE:
• Proven experience within a medical setting at band 6 or above
• Recent experience of working with respiratory patients within primary/secondary care and the ability to transfer these skills to the appropriate setting
• Clinical knowledge in respiratory patient assessment and care
• Experience of the diagnosis and management of respiratory conditions
• Experience of supervising staff/students
• Experience of effective team working and working as part of a MDT
• Experience of educating patients in self-management of their condition and devising individualised care plans
